Melissa N Formo, 88, of Mammoth, AZ, passed away to eternal life on October 20, 2023. Melissa was born January 15, 1935 in Victor,Utah to parents Hyrum and Martha (Wells) Noyes. Melissa was a beloved mother, grandmother, and great-grandmother.

Melissa was preceded in death by her husband, Bill Formo, grandson Joshua Formo, and great-grandson Jax Formo. She is survived by sons Robert (Julie) Formo and Kiven Formo, 5 grandchildren, Richard (Christa) Formo and their 2 children, Savannah (Jose) Ruiz and their 3 children, Logan Formo and their 2 children, Hyrum (Camille) Formo and their2 children, Kelly Formo and her child and Wyatt Formo. Melissa is also survived by her sisters, Lawanna White, Laura Johns, and Toinette Thomas and a brother, Ronnie Noyes.

A Memorial Service will be held at the Church of Jesus Christ of Latter-Day Saints in San Manuel, Az, Thursday, October 26th at 10:00 AM.
